Aged Care Services


Aged Care Assessment Service

Hours: Monday – Friday, 8.30am – 5pm

Aged Care Assessment Service (ACAS) - ACAS is a multi-disciplinary team who assist older people and their carers to identify what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACAS provides a comprehensive medically based assessment of care needs and information on suitable care options.

Case Management Services (CMS)

Hours: Monday – Friday, 8.30am – 5pm

Case Management Services provides support for aged and frail people, people with a disability or chronic illness, and their carers, to live independently in their local community.  The Case Managers are skilled professionals who use person centred principles to assist clients to identify, achieve and direct their care planning and coordination. Case Managers will also assist to manage client’s care packages. The Linkages Program is supported by funding from the Commonwealth and Victorian Governments under the HACC program.

Dementia Behaviour Management Advisory Service (DBMAS)

Hours: Monday – Friday, 9am – 5pm

The role of the DBMAS program is to improve the quality of life of people with dementia and their carers where the behaviour of the person with dementia impacts on their care. This is achieved by improving the dementia care capacity of care workers, carers and service providers.

Geriatric Evaluation and Management Services (GEM)

Hours: 7 days a week, 24 hours a day

The Inpatient and Evaluation Unit hosts patients admitted for Geriatric Evaluation and Management (GEM) who have chronic or complex conditions associated with aging, cognitive dysfunction, chronic illness or disability. The client group are predominantly, but not limited to older people with a primary treatment goal of maximising health status and optimising living arrangements. The patient and family have access to a multidisciplinary team.

Health Surveillance
Hours: Monday – Friday, 8.30am – 5pm

The Health Surveillance Program (HSP) assists frail older and disabled clients in the Bendigo Community to maximise safety, independence and quality of life in their own homes and communities by providing a home visiting service that monitors clients regularly for early detection of health risks and intervention to prevent illness and hospital admission. Eligible clients are those waiting for an aged care package.

Residential In Reach (RIR)

Hours: 7 days a week, 7am – 3.15pm

RIR is a nurse lead program that provides specialist assessment, coordination, care provision and treatment for residents within aged care facilities. The service provides an alternative to the Emergency Department for the management of acute conditions and aged related issues and applies a person centred approach.

Transition Care Program and Restorative Care

Hours: Monday – Friday, 8am - 4pm

The Transition Care Program (TCP) program facilitates care for older people who have completed their hospital stay and need more time and support to make a decision on their long term aged care options. TCP provides older people with a package of services that includes low intensity therapy e.g. physiotherapy and occupational therapy, as well as social work, nursing support and personal care.

Restorative Care (RC) is a similar program that allows younger people to access the service, from hospital, emergency department and their home. A multidisciplinary package of services is available.

TCP and RC is a regional service and are provided in the community or is bed based. The service is provided to the individual seven days a week.
