You will need a referral letter from your GP or medical practitioner.
When your referral is received it will be reviewed based on clinical need.
We will then notify you of your appointment time or if you have been placed on a waitlist.

Patients are seen strictly in order of appointment time. We do our best to run to time, however delays do occur.

You can provide information using your personal letterhead or a computer generated form. Please include information about the patient’s condition, functional ability, the impact their condition is having on normal daily activities and some indication of the expected outcomes. Your patient will be contacted to organise an appointment time.
A small capped co-facility payment is incurred for services. This will not be charged if it presents a barrier to client’s accessing services.
Patients covered by WorkSafe, TAC and the Department of Veterans’ Affairs will not be asked to pay this fee as the cost will be met by their insurers.
