Carer support planning aims to identify areas of support that best meet the carers needs and develop a short-term plan to assist them in their caring role.
Our Assessment & Planning Officers will chat with the carer about their caring role and together they will develop a plan.
The Assessment & Planning Officer will arrange any referrals or supports as agreed upon, and check in with the carer from time to time to discuss how they are going.
Carers needs vary greatly and can include urgent respite, counselling, peer/group support, assistance in the home, equipment to assist them in their caring role or tools to help them return to work or study, just to name a few.
Each caring situation is unique and carer support planning determines supports that can be introduced to help carers in their caring role and improve their wellbeing.
