Overview and aims of the role

The Cancer Nurse Coordinator (CNC) will provide a leading role in clinical coordination, information and navigation support to new patients referred to the cancer clinics within the Cancer Centre. The CNC will provide a welcoming centralised entry point for all new patients and coordinate cancer disease management from initial diagnosis to treatment in line with optimal care pathways. 

 The role aims to ensure all new patients and their families receive an informed and supported entry to cancer services to assist them to navigate the complex health system. The focus will be on promoting patients' wellbeing, reducing anxiety and empowering patients to ask questions and self-manage where possible.  

Key responsibilities:

• Provide triage support for new patient referrals and work-ups, liaise between relevant healthcare teams to facilitate timely appointments and treatment planning in line with the optimal care pathways (OCPs).
• Provide centralised intake of new patients referred to oncology clinics, including initial phone contact, assess supportive care needs, provide information about cancer centre and supports, and coordinate the next steps of the pathway.
• Identify patients at risk and initiate early referrals to appropriate services and supports appropriate for patient needs. 
• Be the primary contact for patients and families for advice, questions and navigation support.
• Support movement in and out of cancer services for new patients, and patients that require a change of treatment pathway or care at other locations (metro or regional) in a seamless transition between services and providers.
