The clinic is for amputee clients experiencing a deterioration in function or skin breakdown. This clinic provides individual medical assessments, prosthetic management, and full rehabilitation services including occupational therapy, physiotherapy, social work, dietetics, and podiatry.

How to access this clinic

You will need a referral letter from your GP or medical practitioner.
When your referral is received it will be reviewed based on clinical need.
We will then notify you of your appointment time or if you have been placed on a waitlist.

What to bring

  • Appointment letter and registration form
  • Medicare card
  • List of medications that you are currently taking
  •  Any relevant test results or x-rays from services outside Bendigo Health
  • Any relevant cards (Health Care Card, Aged Pension, Department of Veterans' Affairs)

How to refer your patient

Refer via e-Referral (SeNT), fax or post to the Bendigo Health Referral Centre.

You can provide information using your personal letterhead or a computer generated form. Please include information about the patient’s condition, functional ability, the impact their condition is having on normal daily activities and some indication of the expected outcomes. Your patient will be contacted to organise an appointment time. 

Cost

A small capped co-facility payment is incurred for services. This will not be charged if it presents a barrier to client’s accessing services.

Patients covered by WorkSafe, TAC and the Department of Veterans’ Affairs will not be asked to pay this fee as the cost will be met by their insurers.
