Community Nursing Services

Our service provides home-based nursing care to support people to live at home safely and independently and remain active members of the community.

What we do

We offer a range of district nursing services to support clients in their home:

  • Comprehensive health assessment
  • Goal setting and care planning
  • Clinical care such as wound care, medication support, catheter care, PEG tube and stoma care

We provide a nurse consultancy service in the following specialty areas:

  • Diabetes
  • Dementia and cognition
  • Wound care
  • Stomal therapy
  • Continence

We can help you with:

  • Referral to other services to support your goals
  • Providing health information to facilitate recovery and wellness

Who we care for

Our service is available to people who require care to be delivered in their home when activities of daily living are impacted. Please phone our office if you have any questions about eligibility for our service.

We also deliver services to National Disability Insurance Scheme (NDIS) participants, Home Care Package holders, Transport Accident Commission (TAC) and Department of Veteran’s Affairs (DVA).

District nursing service is available to people living in the Bendigo area within a 30km radius of the CBD. Services may be negotiated for clients outside this area.

Regional Community Continence Service is provided in the City of Greater Bendigo, Loddon, Buloke, Campaspe, Swan Hill and Gannawarra shires.

Regional Dementia Consultancy Service is available throughout the Loddon Mallee Region. This can be provided by phone, telehealth, or face-to-face.

Referrals can be received through My Aged Care or Bendigo Health Referral Centre [email protected]

How to refer your patient?

Client are required to pay a subsidised fee. Client consent is required for all referrals.

Referrals for clients aged over 65 are received through My Aged Care, if the service is urgent a referral through the Bendigo Health Referral Centre [email protected] is also required.

Referral for clients aged 64 and under are received through the Bendigo Health Referral Centre [email protected].

  • Commonwealth Home Support Program (CHSP) services (people aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people)
  • Home and Community Care Program for Younger People (HACC PYP) services (people aged under 65 years or 50 years and under for Aboriginal and Torres Strait Islander people)
  • National Disability Insurance Scheme (NDIS) participants, Home Care Package holders, Transport Accident Commission (TAC) and Department of Veterans’ Affairs (DVA)
