1. What is the Healthy 4 U (H4U) Program?

  • The H4U Program is an evidence-based health prevention and promotion initiative designed to empower individuals in rural and regional Victoria to make positive changes in physical activity, dietary choices, and smoking habits. It utilises motivational interviewing and cognitive behavioural therapy principles for effective support.

2. Who is eligible to participate in the program?

  • The program is tailored for ambulatory patients considering, or booked in for, future surgeries, residing in rural and regional areas of Victoria within the Loddon Mallee region.

3. How does the program work?

  • Participants engage in up to six 20-minute telephone coaching sessions led by experienced health promotion professionals. These sessions incorporate motivational interviewing and cognitive behavioural therapy, providing personalised guidance to enhance physical activity, improve dietary choices, quit smoking, and boost overall well-being.

4. When does the program run?

  • The H4U program will run from the 1st of October 2023 to the 30th of June 2024 and coaching sessions will conducted between 8am and 5pm on Mondays and Tuesdays.

5. What are the program objectives?

  • The program aims to increase physical activity levels, improve nutrition and diet choices, enhance self-efficacy, examine healthcare utilisation, foster community engagement, and evaluate the effectiveness of telephone coaching as a tool for improving health outcomes in rural and regional Victoria.

6. How do I register for the program?

  • Registration can be done by calling or texting ‘Yes’ to our dedicated program mobile contact number (0439 412 329). A comprehensive orientation call will guide you through the initial steps and allow you to schedule your first coaching session at a time that suits you.

7. Is there a cost to participate?

  • No, the H4U Program is generously supported by the Department of Health Victoria, ensuring that participants can access high-quality health promotion services at no cost.

8. Can I participate if I have a pre-existing health condition?

  • Yes, the program is designed to be flexible and to suit everyone. Your coaching sessions will be guided by you and what you want to focus on.

9. How is the program evaluated?

  • We employ a rigorous evaluation framework, utilising validated tools such as the International Physical Activity Questionnaire, Physical Activity Stages of Change Questionnaire, Fagerström Test for Nicotine Dependence, and The Australian Guide to Healthy Eating 2 Fruit and 5 Veg Questionnaire.

10. What happens after the program concludes?

  • Post-program, participants are encouraged to maintain their trajectory toward better health and leverage the skills and knowledge gained to maintain a healthier lifestyle.

  • Currently, the program is focused on the Loddon Mallee region, we recommend exploring local health promotion programs in your area or staying tuned for potential program expansions.

13. How often are the coaching sessions, and what if I miss one?

  • Participants engage in up to six 20-minute telephone coaching sessions which are booked at times that suit you. If you miss a session, don't worry—your health coach will work with you to reschedule and ensure you receive the full benefit of the program.

14. Are the coaching sessions confidential?

  • Yes, confidentiality is a priority in the H4U Program. Your personal information and discussions with your health coach are kept confidential in accordance with privacy regulations.

15. Can I participate if I have mobility limitations or other physical challenges?

  • Absolutely. The program is adaptable to various physical abilities, and your health coach will work with you to tailor recommendations based on your individual circumstances.
